These SMS/Text Messaging Terms and Conditions (the "Terms") govern the SMS program provided by Lewis Pain & Physical Medicine, operated by Jerry W. Lewis MD PA ("Lewis Pain," "we," "our," or "us"). By opting in to or using the SMS program, you agree to these Terms. If you do not agree, do not enroll in or use the program.
1. Program Description
The Lewis Pain SMS program supports communications related to your interaction with our practice. Depending on your relationship with us, messages may include:
- Appointment confirmations, reminders, scheduling changes, and office updates.
- Patient-service or care-coordination communications and responses to inquiries.
- Account, billing, or other administrative notices related to services you receive from us.
The program is not used to share or promote third-party products or services.
2. Message Frequency
Message frequency varies based on your appointments, requests, and interactions with us. Messages may be recurring while you remain enrolled.
3. Message and Data Rates; Carrier Disclaimer
Standard message and data rates may apply according to your mobile service plan. Your mobile carrier determines those charges. Wireless carriers are not liable for delayed or undelivered messages.
4. How to Opt In
You may opt in by checking the affirmative SMS consent box on our Contact Us form and submitting your mobile number. Where keyword enrollment is available, you may also text START to 214-618-9600. The consent box must be selected by you and is not preselected. Consent to receive SMS messages is not a condition of purchase or receipt of care.
5. How to Opt Out
Reply STOP to any message to opt out at any time. We may send one final message confirming that your opt-out request was processed. After that confirmation, you will not receive additional messages from that messaging program unless you opt in again. You may opt back in, where supported, by replying START.

7. Privacy and Use of Mobile Information
Mobile information and SMS opt-in data and consent will not be sold, rented, or shared with third parties or affiliates for their marketing or promotional purposes. Vendors that help us operate the SMS program may process this information only to provide those services for us and may not use it for their own marketing or promotional purposes. 8. Your Responsibilities
You represent that you are the subscriber or customary user of the mobile number provided and are authorized to enroll that number. Notify us if you change or deactivate the number. Do not use SMS for emergencies or urgent medical needs. Call 911 in an emergency. Standard SMS is not guaranteed to be secure, and you should not include unnecessary sensitive medical information in a text message.
9. Changes to These Terms
We may update these Terms from time to time. Revised Terms will be posted on our website with an updated date. Changes become effective when posted unless otherwise stated or required by law.
